Key Responsibilities:

  • Conduct comprehensive assessments and evaluations of patients with complex wound care needs.

  • Develop personalized care plans tailored to each patient's wound management requirements.

  • Perform advanced wound care interventions including, but not limited to, debridement, dressing changes, and use of negative pressure wound therapy.

  • Collaborate with interdisciplinary teams to ensure a holistic approach to patient care.

  • Educate patients and families about wound healing processes and preventive measures.

  • Document all clinical findings and care provided in accordance with company policies and regulatory requirements.

  • Stay updated on the latest advancements in wound care therapies and practices.

  • Participate in patient rounds and case discussions, providing insights into wound management strategies.

Essential Qualifications:

  • Current Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of California.

  • Wound care certification (Wound Certified) is mandatory.

  • Master's degree in Nursing, Physician Assistant studies, or a related field.

  • Strong clinical skills in wound care and management.

  • Excellent verbal and written communication skills.

Desired Experience:

  • At least 2 years of clinical experience in wound care management.

  • Experience in an interdisciplinary healthcare setting.

  • Familiarity with current wound care technologies and treatment modalities.

  • Previous experience in home health care, hospice, or palliative care is a plus.
