Job Responsibilities:

  • Conduct thorough assessments and evaluations of patients with complex wound care needs.

  • Develop and implement individualized care plans based on best practices and clinical guidelines.

  • Collaborate with a multidisciplinary team to provide comprehensive wound management.

  • Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.

  • Educate patients and their families on wound healing processes, treatment options, and prevention strategies.

  • Document patient care accurately and timely in compliance with healthcare regulations.

  • Participate in staff meetings, training sessions, and continuing education opportunities to enhance skills.

  • Stay current with the latest advancements in wound care and relevant technology.

Essential Qualifications:

  • Graduation from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.

  • Current and valid NP/PA license in the state of practice.

  • Wound Care Certification is required.

  • Strong clinical assessment skills and the ability to work independently.

  • Excellent communication and interpersonal skills.

  • Strong organizational skills and attention to detail.

Desired Experience:

  • Minimum of 2 years of experience in wound care or related field preferred.

  • Previous experience in a clinical setting focusing on advanced wound management.

  • Experience working in home health care, hospice, or palliative care settings is a plus.
