Job Responsibilities:
Conduct comprehensive assessments of patients' wound care needs.
Develop and implement individualized care plans for wound care management.
Perform advanced wound care treatments and procedures.
Monitor and document patient progress and outcomes consistently.
Educate patients and their families on wound care and preventive strategies.
Collaborate with interdisciplinary teams to ensure holistic patient care.
Maintain compliance with all regulatory and safety standards in wound care.
Stay updated with the latest wound care trends and practices.
Essential Qualifications:
Current and valid RN license in the state of California.
Wound Care Certification (WC-C), demonstrating advanced knowledge in wound management.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work effectively in a fast-paced environment.
Desired Experience:
Minimum of 2 years of experience in clinical nursing, with a focus on wound care.
Experience working collaboratively within a healthcare team.
Prior experience in home health or hospice care settings is a plus.
