Job Responsibilities:

  • Provide direct patient care to individuals with complex wound care needs.

  • Conduct assessments to evaluate wound conditions and objective findings.

  • Develop and implement personalized care plans collaboratively with the healthcare team.

  • Educate patients and families on wound care management and prevention strategies.

  • Perform treatment procedures including debridement, dressing changes, and wound assessments.

  • Document care provided and monitor progress, adapting care plans as necessary.

  • Stay updated on emerging practices and innovations in wound care management.

Essential Qualifications:

  • Current RN license in the state of California.

  • Wound Care certification (WCC, CWS or equivalent).

  • Knowledge of current evidence-based practices in wound care management.

  • Strong communication and interpersonal skills.

  • Ability to work both independently and collaboratively within a healthcare team.

Desired Experience:

  • Minimum of 2 years of nursing experience, with a focus on wound care preferred.

  • Experience in assessing and managing patients with chronic wounds.

  • Prior experience in home health, hospice, or palliative care settings is a plus.
