Job Responsibilities:
Provide direct patient care to individuals with complex wound care needs.
Conduct assessments to evaluate wound conditions and objective findings.
Develop and implement personalized care plans collaboratively with the healthcare team.
Educate patients and families on wound care management and prevention strategies.
Perform treatment procedures including debridement, dressing changes, and wound assessments.
Document care provided and monitor progress, adapting care plans as necessary.
Stay updated on emerging practices and innovations in wound care management.
Essential Qualifications:
Current RN license in the state of California.
Wound Care certification (WCC, CWS or equivalent).
Knowledge of current evidence-based practices in wound care management.
Strong communication and interpersonal skills.
Ability to work both independently and collaboratively within a healthcare team.
Desired Experience:
Minimum of 2 years of nursing experience, with a focus on wound care preferred.
Experience in assessing and managing patients with chronic wounds.
Prior experience in home health, hospice, or palliative care settings is a plus.
