Job Responsibilities
Provide comprehensive wound care assessments, planning, and intervention to ensure optimal patient outcomes.
Collaborate with healthcare teams to develop individualized care plans.
Monitor and evaluate patient progress, adjusting treatment plans as necessary.
Educate patients and families on wound care and prevention strategies.
Document patient care activities and progress accurately and promptly in electronic health records.
Participate in quality improvement initiatives and interdisciplinary team meetings.
Stay updated with the latest research and advancements in wound care management.
Essential Qualifications
Current Registered Nurse (RN) license in the State of California.
Wound Care Certification (WCC or CWS preferred).
Strong clinical skills in wound management and patient care.
Effective communication and interpersonal skills.
Ability to work collaboratively within a multidisciplinary team.
Desired Experience
Minimum of 2 years of clinical nursing experience, with a focus on wound care preferred.
Previous experience in home health, hospice, or palliative care settings is a plus.
Proficiency in electronic health records and documentation processes.
